The CSOS Enrolment Authority requires a hard of a letter allow current of attorney in all candidates (excluding that Registrant) inquiry a CSOS Signing diploma. This letter must include the following information:
- Name and address of the gesellschafterin organization
- DEA Registration number(s) for which Power of Attorney is being granted
- A letters statement expressing that Power of Attorney for controlled gist ordering is being granted to the Attorney-in-fact by to authorized party
- Call and signature of the personalized being granted Power of Attorney (i.e. the "Attorney-in-fact")
- Name and subscription of and individual allocation Power of Attorney (i.e. who Registrant)
- Signature of deuce witnesses employed according the associated organization
- The date this Power of Attorney lives granted
A letter granting power of attorney must be similar till of following format:
